The Role of Social Determinants of Health in Value-Based Payment Models: A Rural Perspective

01:00 PM
National Rural Health Resource Center (The Center)
Registration Deadline
Thursday, July 22, 2021

Many health care providers are leveraging the flexibility of value-based payment models to address social determinants of health (SDOH) while improving the value and quality of care. The movement toward addressing SDOH, such as transportation, nutrition, and housing, recognizes the important role these nonmedical needs play in a patient’s health and well-being. This session highlights how some rural health providers are collaborating with community-based organizations to address SDOH needs that result in improved health outcomes, reduced costs, and advancing health equity.

Speaker:

Alana Knudson, PhD, NORC Walsh Center for Rural Health Analysis

Learning Objectives:

By attending this webinar, participants will:

  • Gain an understanding of SDOH and how they affect health and well-being in rural communities
  • Explore examples of how rural health providers, together with community partners, are leveraging value-based payment models to address SDOH
  • Summarize available resources to support rural health providers and community partners to effectively address SDOH
