We are trying to submit the Hybrid Hospital-Wide Readmission measure for the first time. I understand the Core Clinical Data Elements need to also include 6 Linking Variables for each patient so CMS can merge the EHR data elements with the claims data elements. What format do the linking variables and the other Core Clinical Data Elements need to be in to ensure CMS is able to link the claims data elements?

In previous newsletters it was mentioned there is a Sex Assigned at Birth data element change that will impact data submission starting with Q3 2024 encounters for the Median time from ED arrival to ED departure measure (OP-18). When there was a Sex element change in 2023, Quality Net released a new version of the CMS Abstraction & Reporting Tool (CART) to handle this change. For CART users, will there be another CART update to handle this Sex Assigned at Birth data element change?  

I have been reviewing my hospital's HCAHPS scores and am looking for more information on HCAHPS procedures and opportunities for training. Where can I find that information?
