Small Rural Hospital Transition (SRHT) Project
The Small Rural Hospital Transition (SRHT) Project supported small rural hospitals nationally by providing on-site technical assistance to assist in bridging the gaps between the current health care system and the newly emerging health care delivery and payment system. From 2014-2019, 52 Quality Improvement and/or Financial Operation Assessment projects were supported through SRHT.

SRHT selected hospitals participated in either a financial operational assessment (FOA) or a quality improvement (QI) and transition of care project. The onsite consultations were designed to assist hospitals with:
- Improving financial and quality performance
- Preparing for new payment and care delivery models
- Preparing to transition to a system based on value
