Care Coordination is a part of every Path to Value project. Effective care coordination plays a key role in effectively meeting the needs of an intended population and achieving organizational measures. It also plays a large role in enhancing a hospitals’ finances. The panelist from this webinar will talk about how the intended population has been affected along with the hospital’s bottom line. 

Objectives: 

  • To understand how care coordination is an essential component of ACOs and other value-based payment models.
  • To understand how a full range of care coordination services can positively impact the hospitals financial bottom line.  
  • To understand which of the care coordination services are currently reimbursable, and which services will lead to greater patient volume and enhanced revenue.
